In connection with your participation in the AIA National Lecture Program, please complete the following form.

Please sign below if, to the best of your knowledge, you can affirmatively attest to each of the following statements:

  • I have never been the subject of an adverse finding by a court, university or other institution of higher education, employer, governmental agency or tribunal, professional organization, or any adjudicatory body in an investigation, complaint, or proceeding regarding allegations against me of discrimination, sexual misconduct, assault, harassment, or any act of moral turpitude.
  • I am not currently the subject of an investigation, complaint, or proceeding by a court, university or other institution of higher education, employer, governmental agency or tribunal, professional organization, or any adjudicatory body for discrimination, sexual misconduct, assault, harassment, or any act of moral turpitude.
  • I have never resigned, retired, or otherwise separated from a university or other institution of higher education, professional organization, or place of employment during or as a result of an investigation, complaint, or proceeding in which I was accused of discrimination, sexual misconduct, assault, harassment, or any act of moral turpitude.
  • I have never been party to a legal settlement arising out of allegations accusing me of discrimination, sexual misconduct, assault, harassment, or any act of moral turpitude.
